There are 2 main reasons why homeless people get this type of necrosis:

  1. freezing cold, destroys cells, ruptures membrenes and causes vascular collapse and ischemia due to blood clotting, oxygen deprivation and reperfusion injury which results in necrosis
  2. xylazine and other potent vasoconstrictors that people take to make drugs last longer such as fentanyl, restricted bloodflow also causes this type of necrosis.
  3. a combination of the 2 vasoconstrictors prevent warm blood from circulating, making extremities freeze much easier/faster.
